Bulk-scan UX: parallelizable OCR against evidence matrix

Problem Statement. BulkScannerScreen (referenced in AppNavigator) processes QR
codes one-at-a-time. Field operators scan dozens of items per minute.

Technical Context.
app/mobile/src/screens/BulkScannerScreen.tsx.

Expected Outcome. Up to 4 concurrent in-flight scans; deduplicated results; rate
limit handled.

Acceptance Criteria.

• A test asserts throughput ≥ 2× current on a sample of 50 items.
