Tried disabling ZHA, rebooting and then trying to install. Still no luck.

Lamp is no showing

ZHA and Zigbee devices are working well - and migration worked as expected

Tried installing second Adaptor - to run Thread protocol - it has been detected as a USB device - but is not showing on HA - not sure if it is related to the first adaptor update failing.

There will be a new version of Core released in about an hour. Give it a try again after upgrading Core.

Just to check back in, Home Assistant Core 2025.11.3 is now released with a fix for most/all flashing problems.
If you keep experiencing issues, you can also try the web-flasher at: https://toolbox.openhomefoundation.org/home-assistant-connect-zbt-2/install/

In another issue, someone also mentioned that power-cycling the host (and ZBT-2) may help.

If the discovery entry in HA is missing, try restarting HA Core and see if it appears then. We'll make this more resilient in future HA versions.
