Intermittent "Error: Unknown error" messages appear as separate bot messages in Telegram immediately after a normal, successful bot response. The actual response delivers correctly — the error shows up as an additional phantom message bubble.
Reproduction
- Happens multiple times per session, intermittently
- Occurs after both plain text responses AND tool-call responses
- No specific trigger — even simple text replies with zero tool calls produce it
- App logs show no errors at the time of occurrence
