1. Update the installation.md with examples
2. Capture [FEATURE] add a managed-by label to CRD #438 guidelines once implemented
3. Update stale charts/node-readiness-controller/README.md

Picked this up, PR is #388.

Helm was not in the book at all, so it adds a section to the install page with the values for metrics, TLS and the webhook, plus the CRD upgrade and uninstall gotchas.
