perf(cuda): preclusion-layer 2:1 balance #9

Replace the serial ripple with a single-round insulation/preclusion-layer balance ([IBG2012]) on the GPU. Active-front balance already shipped (1.84x sphere); preclusion is the next step. Validate byte-identical against balance_ref().
