. Add course enrollment waitlist

What: Add a waitlist feature for courses that have reached their enrollment cap.

Why: If enrollment limits are implemented (issue #39), users who can't enroll need a way to be notified when a spot opens. A waitlist provides this functionality.

Scope: Create enrollment_waitlist table with user_id, course_id, position, created_at. Add POST /api/v1/courses/:id/waitlist and DELETE /api/v1/courses/:id/waitlist endpoints. When an enrollment is dropped, notify the next person on the waitlist.

Acceptance criteria:

Users can join a course waitlist
Users can leave a waitlist
Position is tracked and returned
Next person is notified when a spot opens

Technical context: src/modules/courses/course.service.ts — enrollment logic. src/database/schema.ts — schema definitions.
