fix: EU에 table_index 보존 및 분할 마지막 조각 page_span 누락 수정 Fixes #25 - #29

- chunker.py에서 계산만 하고 버리던 table_index를 EvidenceUnit 필드로 저장. QA 생성기가 이미 meta.table_index로 정답 표를 태깅하고 있어 조인 키로 씀. (페이지 일치만으론 같은 페이지의 다른 표와 구분 불가)
- split.py: 표가 행 단위로 쪼개질 때 마지막 조각만 page_span 전파가 빠져 있던 버그 수정(앞쪽 조각들은 정상)
