Harden Summarizo PDF selection and adaptive summary context use

Use cached leading full-text headings to disambiguate close-scoring Zotero PDF
attachments, demoting article-like supplemental files only when metadata alone
cannot clearly identify the primary PDF.

Load summaries through calibrated LLM load plans, start large auto-context
models at a safer half-context, retry decode resource failures at smaller
contexts, and split oversized summary chunks recursively when the engine reports
insufficient generation budget.
