Test-first RED diagnostic, based on exact PR #92 commit b77ad0a. H76: GET /v1/scale-context must not unlink terminated-bootstrap staging hard links during read-only V2 preflight. H77: index.db witnessing a later acknowledged event must block rolled-back ledger+snapshot read-only projection and loadStateSnapshot. Tests committed before source changes. Not a release integration candidate, independent review, acceptance or merge authorization.
