fix(responses): preserve adapter body stall status - #8200
A streamed adapter response-body timeout carried status 504 into the bridge, but its terminal was recorded as 502 and lost the bounded body-stall duration. Emit the existing upstream_stall_timeout code and the guard's bounded duration message, and preserve the same message on buffered timeouts. The request log now classifies the timeout as 504 and retains body_stall; ordinary upstream failures remain 502. Already-committed SSE HTTP headers are unchanged.
Document the adapter-body timeout separately from the bridge watchdog in the public Responses architecture reference, including stream commitment and buffered behavior.
Reference: RFC 9110 §15.6.5 defines 504 for a gateway that did not receive a timely upstream response. The existing response-body inactivity guard and bounded failure-detail classifier supply the local contract. Aside was unavailable on the Linux host; the hosted search tool returned 401, so the RFC was fetched directly.
Verification
bun test tests/server/error-fidelity.test.ts tests/server/terminal-guard-server.test.ts — exit 0, 28 passed. The streamed case exercises the actual terminal and request-log classification; the buffered case checks HTTP 504 and the bounded duration. A normal upstream error remains 502.
Before the fix, bun test tests/server/error-fidelity.test.ts -t 'a structured upstream stall' failed with 502 instead of 504, and bun test tests/server/terminal-guard-server.test.ts -t 'a stalled initial body' failed on the generic code and replacement message.
bun run typecheck, bun run structure:check, bun scripts/file-size-ratchet.ts, bun run privacy:scan, and git diff --check — exit 0. In docs-site/, bun install --frozen-lockfile and bun run build — exit 0; the build checked 93,150 internal links across 641 pages.
bun run test:changed — exit 1: 50,199 passed, 236 skipped, 12 failed across 2,240 files. Each affected file was run separately on this head and clean origin/devf202b2d40 using bun scripts/test.ts <file>:
The full bun run test was not run locally because six lanes run concurrently; broader coverage is left to CI. Prior CI belongs to the previous head and is not evidence for this one.

Summary by CodeRabbit
Bug Fixes
Adapter response-body inactivity timeouts now consistently return HTTP 504 with a bounded-duration message.
Streamed timeout failures include the upstream_stall_timeout code, while existing stream headers remain unchanged.
Timeout details are recorded as body_stall; ordinary upstream failures retain their existing classification.
Documentation
Updated architecture and error-detail references to describe timeout behavior and classification.
Adapter body-inactivity errors preserve the inactivity message. Streamed timeout errors include upstream_stall_timeout, which terminal classification maps to HTTP 504. Tests and documentation cover streamed and buffered timeouts and body_stall details.

Streaming and recovery timeout errors use the inactivity error’s message and include upstream_stall_timeout. Terminal classification maps that code to HTTP 504. Buffered timeout responses retain HTTP 504 and use the inactivity message. Tests check streamed and buffered status classification and body_stall detail value 1000. Documentation describes these timeout classifications.
lidge-jun/opencodex#8104: Adds the failureDetail projection that records the body_stall detail preserved by this change.
lidge-jun/opencodex#7472: Changes thinking-signature recovery stall-timeout handling in adapter-delivery.ts, which this change updates to preserve the inactivity message and timeout classification.

Continuation stalls can still be logged as 502 or returned as HTTP 200 despite the new documentation. The change is mergeable with awareness of these exceptions.

Add a recovery-stall regression test.
In tests/responses/responses-anthropic-thinking-signature-recovery.test.ts, return a stalled body after a streamed signature rejection. Assert the terminal error code and its 504 classification. Without this test, removing the recovery event’s code would leave the focused stall tests passing while logs classify the timeout as 502.

Assert the final request-log entry for the stalled stream
The test classifies the SSE failure in a separate context. It can pass if the deferred logger records the wrong status or detail.
Pass the handler’s context and response through responseWithDeferredRequestLog, capture its entry, and assert status 504 and { code: "body_stall", value: 1000 }.

Preserve the timeout in terminal-guard continuations.
When a continuation body stalls, src/server/responses/adapter-continuation.ts:751-752 discards the duration and timeout code. The bridge emits upstream_server_error, so the request log can record 502 without body_stall. This path predates the PR, but it remains outside the stated streamed body-stall fix.
Preserve the same fields as the initial-stream handler, and update the continuation test to assert the terminal code, logged status, and detail.

Return HTTP 504 for buffered continuation body stalls.
When a terminal-guard continuation stalls, src/server/responses/adapter-continuation.ts:752 emits a 504 error event. buildResponseJSON discards its HTTP status, and src/server/responses/adapter-delivery.ts:412 returns the failed response with HTTP 200.
Carry a distinct body-stall indicator and its status through the buffered fold. Use that status when constructing the response, without changing other buffered error statuses. Add a buffered continuation-stall test that asserts HTTP 504 and a failed response.
