Steps to reproduce (REQUIRED)

1. Install context-mode 1.0.169 (npm latest), plugin: ["context-mode"] in opencode.json
2. Fetch any URL whose HTML contains an empty table, e.g. GitHub repo pages render empty markdown tables as
3. Minimal repro: call ctx_fetch_and_index on any page containing — turndown 7.2.4 + turndown-plugin-gfm 1.0.2 throws TypeError, the whole fetch fails
4. Control: without the gfm plugin, converts fine

What have you tried to fix it?

• Read the source: turndown-plugin-gfm 1.0.2 rules.table.filter calls isHeadingRow(node.rows[0]) without guarding empty tables — rows[0] is undefined when the table has no rows
• Isolated from a real GitHub page: an empty (21 chars) alone reproduces the crash; minimized to (16 chars)
• Verified context-mode has no try/catch around the turndown conversion (src/server.ts HTML branch), so the exception escapes as the raw stack
• Searched existing issues: no prior report (only Windows: boot-time turndown/better-sqlite3 npm install fails EPERM (C:\Windows\node_modules) every session — shell:true drops cwd #861/fix(windows): run the better-sqlite3 boot-install via node, not npm.cmd+shell (#861 follow-up) #863 about turndown install on Windows, unrelated)
