fix(web): pull requests error names the environment that failed - #27882
When one environment the Pull Requests page reads from is unreachable (for example a T3 Connect machine that is shut down), the page shows a warning such as "Relay could not reach the environment endpoint (endpoint_request_failed). Showing the last pull requests loaded." It does not say which environment failed, so it reads as if pull request loading is broken in general, and pressing Retry brings the same unexplained banner straight back.
Reproduced on main (26dc368) in the web app. Server A (local, many open PRs) was paired with a second server B, each with one project, and the PR page was opened with all servers. B was then stopped and a new search started. A's rows still listed, but the banner gave only B's transport error with no environment name, and Retry showed the identical banner again.
Change
createMergedEnvironmentQuery in apps/web/src/state/pullRequests.ts used to keep only the first failure's message. It now keeps that failure as { environmentId, message }. The Pull Requests page turns the id into the environment's label and shows <label>: <message>, in both the warning banner (rows shown) and the "Could not load pull requests" state (no rows shown). This is the same format and the same "Environment" fallback the mobile PR list already uses. Which environments are queried and what Retry does are unchanged.
apps/web/src/state/pullRequests.test.ts › "names the environment whose list failed while the others' rows still show": fails on main (the error is a bare string with no environment) and passes with the fix. The file passes 21/21.
vp run check:changed passes on the head (format, lint, knip, typecheck, tests, react compiler).
Takeover
No code changes; evidence and verification added below. The earlier evidence (captured at bcf47b6896 against 26dc3688e2, with GitCafe-only attachments and no scripts) is replaced by a fresh matrix captured at the current head.
Evidence
Before = cb9629e689 (merge-base), After = c60e8ba864 (head). Fixture matrix: matrix.md (9 rows). Full gallery with every checkpoint, sheet and diff: index.html.
Environment: Linux x86_64, headless Chromium 148.0.7778.96 over CDP (agent-browser), no device emulation, DPR 1, 100% font scaling, reduced motion, Date frozen at 2026-11-03T04:00:00Z, sidebar collapsed, scroll origin. REAL rows: two dev servers on fresh databases (T3CODE_DEV_SEED=0); A (fs-32) on pingdotgg/t3code, B (labelled Remote B — issue 27821) on a separate synthetic repo, both loaded and targeted, then B stopped. These are direct connections, not T3 Connect. HARNESS rows mount the real PullRequestsPage with controlled per-environment results for states the app cannot be seeded into.
Diffs: Diff0 = absolute RGB delta ×16, threshold 0. DiffT = magenta where max channel delta >16/255.
P09 (HARNESS, 1440×1000 dark): A, B, C targets; C fails first ~0.1 s; B fails later with blank message ~1.6 s; B recovers ~4.3 s; C fails with a non-Error cause ~7.0 s. The prefix follows the first failing target. before-P09.mp4 · after-P09.mp4
What else changed (beyond the added <label>: prefix):
Warning with rows (P01, P07): text wraps differently; banner height and Retry position unchanged at 1440. At 390 (P03) the long label adds two lines: banner grows 32px, Retry moves down 16px.
Centred error (P02, P08): description gains one line; icon and title move up 10px, Retry down 10px. At 390 (P04): two lines, 20px each way.
P05/P06: missing and empty label maps render byte-identically (Environment:). P06 also changes a few title glyph pixels, consistent with fractional horizontal centring of the wider block.
Healthy and recovered checkpoints (P07, P08): zero changed pixels at threshold 0. No settled comparison changes pixels outside its error region (counts in the manifest).
Mode: evidence-only round; no code, test or review changes on this round. Prior round reviews: both reviewers MERGE.
Tests: apps/web/src/state/pullRequests.test.ts 21/21 (regression test fails on main), unchanged since the earlier round.
Proof audit pass 1: 9-row matrix (P01–P09, 3 recordings). Pass 2: SUFFICIENT for all rows; every capture, diff and recording frame inspected; an independent rebuild of P03–P06 at the head matched the delivered captures with zero changed pixels.
CI on c60e8ba864: 41/41 checks passing.
Not verified: the T3 Connect relay transport itself (no VM has T3 Connect; a stopped directly paired server takes the same page path). Desktop wraps this page; mobile is unchanged and already names the environment.
Evidence: GPT-6.1-Sol (proof audits, captures) in Codex; lead Claude Opus 5.5 in Claude Code, both through T3 Code.
When one environment in the pull requests fan-out fails, the merged list
reports only the bare error message, so the page cannot say which
environment it came from. This test reproduces #27821.
Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
When one environment in the pull requests fan-out failed, the page showed
only the bare error, so an offline machine looked like a page-wide failure.
The merged environment query now reports the failing environment id with the
message, and the page prefixes it with that environment's label, matching
mobile ("archlinux: Relay could not reach ...").

Macroscope's review found this PR approvable — Pull-request loading errors now name the environment that failed in both error displays. Querying, retries, and healthy environments’ rows remain unchanged.

The merged pull request query now retains the ID of the environment that failed. The Pull Requests page uses that ID to prefix the error message in both the empty-list unavailable state and the warning shown alongside existing rows.
The merged query exposes the first failure as an environment ID and message. A test checks that successful rows remain available when another environment fails and that the error includes the failed environment and message.
Show environment context in page errors apps/web/src/components/pullRequest/PullRequestsPage.tsx
The empty-list unavailable state and the warning alongside existing rows now display the error with the environment label, or “Environment” when no label is available.
The requested UI behavior is implemented correctly; adding page-level regression tests is recommended but does not block merging.

Explanation
The title "fix(web): pull requests error names the environment that failed" matches the main change. The diff makes the Pull Requests page prefix errors with the failing environment's label, in both the warning banner and the empty-list error state. The title is specific and follows the conventional commit format the template suggests.
Full details: Description check
Explanation
The description covers the problem, the change, scope and approval, and verification. The problem section gives reproduction steps and the observed banner text. The change section explains the new { environmentId, message } error shape and the label-plus-message display. The scope section links issue #27821 ("Fixes #27821"). Verification names a regression test that fails on main and passes with the fix, and a CI result. It also lists what was not verified, which is the T3 Connect relay transport. Its evidence section is long, with many images, but it meets the template's UI-change requirement for before/after screenshots. Minor gaps: the scope section does not give a maintainer approval comment, and the description does not contain the model and harness line the template asks for in the verification section, although the verification log names the agents used. Those gaps do not make the description largely incomplete, so the check passes.
Full details: Linked Issues check
Explanation
Directly linked issue [#27821] asks that the Pull Requests page name the environment that failed when one environment in the fan-out is unreachable. The PR meets this objective in code:
createMergedEnvironmentQuery (apps/web/src/state/pullRequests.ts) now stores { environmentId, message } for the first failure, instead of a bare string. The environmentId comes from the failing target.
PullRequestsPage.tsx builds listError as ${environmentLabels.get(environmentId) || "Environment"}: ${message}.
listError feeds both the warning banner (rows shown, listError && shownCount > 0) and the PullRequestsUnavailableState (no rows, listError && shownCount === 0).
The issue's suggested behaviour names the failing environment as its first option. This matches the "and/or" wording.
The PR leaves these behaviours unchanged: the set of queried environments, Retry (listQuery.refresh()), and the per-environment offline-status and query-exclusion alternatives. The issue describes Retry and the picker workaround as current symptoms. It presents the offline-status and exclusion ideas as optional ("and/or", "Consider"). They are not stated acceptance requirements, so their absence does not fail this check. Retry still re-queries the offline environment and the banner still appears on Retry. The PR description and objectives state this scope explicitly.
Automated test coverage: the added test "names the environment whose list failed while the others' rows still show" in apps/web/src/state/pullRequests.test.ts is reported by the PR as failing on main and passing at the head. The incremental head 01f19142 is a merge of main. It does not change the PR's own code diff against the merge base.
Full details: Out of Scope Changes check
Explanation
The whole-PR diff against merge base 61c3794c changes only two source files:
apps/web/src/state/pullRequests.ts (+12/-4): the failure record type EnvironmentQueryError and the MergedPullRequestListView.error type. Both serve the naming requirement of [#27821].
apps/web/src/components/pullRequest/PullRequestsPage.tsx (+7/-4): the label prefix and fallback for both error displays. This serves [#27821].
The PR also adds a regression test in apps/web/src/state/pullRequests.test.ts (+53 lines per the summary). It supports the same objective. I did not open the test hunk directly. The evidence images, scripts, and matrix in the PR description are outside the repository and do not change code.
The head 01f19142 is a merge of main. The commit history shows only upstream commits from main in that merge. No unrelated product behaviour or files are changed by this PR. Note for the author: the PR description names cb9629e689 as the merge-base, while the review metadata gives 61c3794c. This does not change the code verdict.
