fix(server): message search no longer finds rolled-back turns or cancelled queued messages - #29874
When a user rolls a thread back, the thread hides every turn after the checkpoint. The message search behind Cmd+K and the agent tool t3_thread_search still returns messages from those rolled-back turns. If you pick one of those hits, the thread opens with in-thread find set to a word the thread no longer shows, so nothing is found and the snippet you clicked is gone. An agent that searches gets hits it can't read back with t3_thread_read.
Cancelled queued messages have the same problem. If you queue a message while a turn runs and then cancel it from the queue (or archive the thread while it is queued), the message never shows in the thread, but search still finds it and the jump lands nowhere. In-thread find already skips these.
The search query already leaves out answers to an agent's question for this reason: find and a jump can't reach them. Rolled-back turns and cancelled queue rows were never excluded. The timeline hides them with createOrchestrationV2TurnItemVisibility, but messageHitsQuery had no SQL equivalent of that check.
Change
messageHitsQuery now skips a message when its run is either:
rolled_back, or
cancelled and no turn item in that run carries the message, other than a queued_turn prompt. The timeline shows messages only through their turn items. A queued message usually has no turn item (an edited one has a queued_turn item), so it stays hidden. A stopped turn keeps its turn_start prompt and its assistant replies, including replies of a run that recovery marked cancelled after it streamed, so search keeps them too.
This is one NOT EXISTS lookup on the runs table's primary key (plus the turn items run_id index for cancelled runs), added next to the existing async-answer: exclusion, so both the FTS path and the scan path apply it. The check runs at query time, which means a rollback or cancellation after a message was indexed is covered without reindexing.
Scope and approval
Very small, focused fix for an obvious bug, so no prior issue. The server's message search now hides the same rolled-back turns and cancelled queued messages that the thread timeline already hides. It is a ~19-line SQL change plus one extended test case.
Overlap search (local PR index plus REST search for "search rolled back", "ThreadSearch rollback", "messageHitsQuery"):
Not covered here: superseded interrupt results, which the timeline also hides. Those are turn items, which message search never reads.
Verification
Red on main: vp test run src/orchestration-v2/ThreadSearch.test.ts -t "rolled-back turns" failed. The search returned the rolled-back reply (threadId: "thread:rewound", snippet "the albatross drive is ready") where it should have returned no matches.
Red for cancelled queued messages, on the first commit of this PR: the extended case failed with expected [ 'stopped-prompt', 'queued-prompt' ] to deeply equal [ 'stopped-prompt' ]. Dropping the preparing-turn exception fails it with expected [] to deeply equal [ 'stopped-prompt' ].
Red for stopped-turn replies, on the second commit: an assistant reply in a cancelled run was hidden, expected [ 'stopped-prompt' ] to deeply equal [ 'recovered-reply', 'stopped-prompt' ]. An intermediate fix that only hid queued_turn items leaked a queued message with no turn item: expected [ 'queued-prompt', …(2) ] to deeply equal [ 'recovered-reply', 'stopped-prompt' ]. Removing the queued_turn clause fails on the edited queued prompt.
Green: ThreadSearch.test.ts passes 26/26.
vp run check:changed: format, lint, knip, graphql, typecheck (3 packages) and tests (2 files, 27) all pass.
Real dev verification now covers Cmd+K rollback, queued cancellation, two-character CJK scan search, stopped assistant replies, and the real Codex agent search tool. The client cache follow-up refreshes both shared search paths on reopening. See the evidence below.

Macroscope's review found this PR approvable — Search now omits rolled-back and cancelled queued messages that cannot be reached in the timeline, and refreshes results when search is reopened. The changes are read-only and narrowly scoped, with no persistent-data or permission changes.

Walkthrough
Thread search now excludes messages from rolled-back runs. For cancelled runs, it excludes messages unless a matching turn item has an inputIntent other than queued_turn. Client search queries now use zero stale time and idle retention.

Cover a reply that was searchable before its run was rolled back.
The fixture creates the run as rolled_back before the reply exists. No test searches a visible reply, rolls back its run, and searches again without reindexing. A filter that applies only at indexing time can pass this test.

Update the test “leaves out answers in their question card and rolled-back

a run.updated event changing the run to rolled_back and search again without
reindexing. Assert the reply is searchable before rollback and excluded

fix(server): message search no longer finds turns that were rolled back

A stopped turn's thread still shows every message an item carries, except
a queued prompt. Search now leaves out only messages of a stopped turn that
no shown item carries, instead of every message without a started prompt.

verified on linux with playwright from the repo and /usr/bin/google-chrome, using real codex gpt-6.1-sol turns. no mocks or seeded fixtures. main ran in a separate worktree at 61c3794c32; only one dev server ran at a time.
main reproduced both bugs: cmd+k returned a rolled-back turn and a removed queued prompt. the pr excluded both against the same app-created data.
fresh turns exposed cached cmd+k hits after rollback on the original pr head. pushed 9f600c6a3e87e9583a3354154d54c4550e3b7271 to the existing contributor branch. both shared client search paths now discard idle results and revalidate on reopening.
after restarting, newly indexed english and two-character cjk matches disappeared immediately after rollback. edited queued messages disappeared after cancellation. visible stopped prompts and assistant replies remained searchable; selecting an assistant-only hit opened in-thread find at 1/1.
the real provider called t3_thread_search four times: rolled-back message 0, removed queue message 0, edited cancelled queue message 0, visible assistant-only phrase 1 (source assistant).
blacksmith vp run check:changed c0ec87a55bbffa437673f037d7b4ca68c6a300f1 passed format, lint, knip, graphql, affected typechecks and 40 tests. latest ci: 24 success, 9 skipped, no failures or pending checks. two independent read-only reviews approved the final head.
native ios/android and electron shells were not run on this linux vm. other providers, relay/tunnel connections, archive-triggered queue cancellation, and crash-recovery cancellation were not independently exercised. shared client typechecks passed; verification used the local web client and codex path.
