…fault CUDA to `IoBinding`. Rename `KvCache` to `KVCache`, add a `CudaKVCache` backend in `cuda_backend.rs`, and keep a single generate loop. CUDA EP now uses device KV by default (`FAST_LIGHTONOCR_CUDA_HOST_KV` to force host).
