Ship Action Center v2 release surfaces

Summary

• ship the v2 Action Center recommendation contract in @xerg/schemas and push payload version 2
• build ranked, stable recommendation objects into core summaries, wire payloads, JSON output, and the CLI Action queue rendering
• bump the synced workspace packages to 0.5.0 and update release-facing docs, README surfaces, and the bundled Xerg skill
